Guide to obtaining out-of-state birth certificates for MassHealth

In response to the federal Deficit Reduction Act (DRA) requirement that people applying for MassHealth document their identity and citizenship, outreach worker Jacqueline Matta of Hampshire HealthConnect created a comprehensive guide to acquiring out-of-state birth certificates. This tool includes links to appropriate birth certificate sources in each state. Birth certificates satisfy the DRA citizenship requirement. Hampshire HealthConnect is a program of Cooley Dickinson Hospital and is partially funded by a grant from the Blue Cross Blue Shield of Massachusetts Foundation.

Update to Birth Certificate docuement

I just wanted to update everyone on an item having to do with the birth certificate form that has been posted onto this website. The number for the Delaware Vital Statistics office is incorrect and the correct number is 302-744-4549.

I've created a simplified guide to DRA-required documents...it is not meant to be all-inclusive, but the idea is that an outreach worker could indicate the needed documents and give it to a client. Click here to see the document.
