Most Massachusetts adults will have to show that they had health insurance on December 31, 2007 or lose their personal exemption. The state Department of Revenue (DOR) has drafted Schedule HC, Health Care Information, for taxpayers to include with Form 1 or Form 1-NR/PY of their 2007 tax return, as well as instructions for its use. Privately insured individuals will receive a Form 1099-HC from their health insurance company which will contain the information needed to complete Schedule HC.
Individuals who do not have insurance, and who believe any insurance available to them is unaffordable, can declare this on page 2. Those in a condition of financial hardship may request a penalty waiver using the Schedule HC-A on page 3.
